Admissions

 

Roosevelt Warm Springs Vocational Unit provides services to individuals who are at least 18 years old, without regard to race, color, national origin, religious creed, sex, or sexual orientation. The same requirements for admission are applied to all referrals. Accessing services of the Vocational Unit begins with a referral for admission. 

Referrals are accepted from Vocational Counselors, Worker's Compensation Providers, and Private Pay Individuals. 

The following information is needed to initiate a referral from Georgia Vocational Rehabilitation Agency Offices:

  • *GVRA Application for Job Placement and Work (Form 001A)
  • *Checklist of supporting documentation
  • *VRU Discharge Plan (Form 169-VRU)


The following information is needed to initiate a referral from Worker's Compensation or private pay individuals:

  • *Application for admission with supporting documentation (Form 388)
